Child travel affidavit

Crossing a border with a child? Make the affidavit.

When a child leaves South Africa with one parent, or with another adult, the parent at home gives consent in an affidavit. Add the trip, the people and their documents, and download the affidavit ready to sign in front of a Commissioner of Oaths.

The trip

When the child leaves South Africa and comes back, and the countries they travel to. These go into the wording of the affidavit.

Countries In the order you travel. The last one is the final destination.

Who is travelling with the child?

Travelling with

Both parents travelling? No affidavit is needed: carry the child's unabridged birth certificate. A court order, a deceased parent or a child travelling alone needs other documents; see the official guidance on gov.za.

Children with different parents? Make a separate affidavit for each family.

Your documents

Add a photo or PDF of each one. They are copied into the affidavit pack, and we read the IDs and birth certificates to fill in the affidavit for you. Phone photos are fine: flat, in good light, all four corners in view.

Your affidavit

Your privacy. Documents are read securely to fill in the forms, then discarded. We do not store them or your details. The PDFs are put together on your own device.

Planning the whole trip? The trip planner makes the itinerary, fuel, papers and packing list, with this affidavit included.
